These are not necessarily all of the items that will be needed for the underwriting of this loan or equity request.

If you are interested in obtaining a business line of credit, please see Question #5 on the form.

Please download either the .DOC form or the .PDF form below and answer each question on this form completely and to the best of your ability. Feel free to copy & paste information from your Executive Summary or Business Plan. Use N/A for any question, you do not have an answer to.

Note that incomplete forms or information will only delay your Managing Director's ability to submit your request to our sources for consideration. The idea is to do what you can to attract the lender's or investor's interest with the information that you are providing here.

Things to Remember:

1. Once you send your information to us, you will be asked to sign an NCND (non circumvent, non disclosure and confidentiality statement) and a fee agreement. Once we receive your NCND and fee agreement, your project will go through a consultation phase, prior to our sending it out to our sources.

2. You will also be asked to forward your Executive Summary and/or Business Plan, etc. as part of the consultation phase ... Our goal is to match you and your project up with one of our sources that has an interest in your project.

3. Note that before we put you on the phone with ANY of our sources to discuss your project (and terms) with you, you will be required to sign an NCND and fee agreement.

4. Important: See question #9d on the form ... We need to know how long you have been looking for funding for your project.
